Damien Center, Inc. proposes the Indy Outreach Navigation (ION) program, which will provide services to those at highest risk for HIV and substance use disorders in Marion County, Indiana, with a focus on Black and LatinX MSM. Utilizing a navigation approach led by a Project Director, two Prevention Navigators, and a Peer Recovery Coach, PNP will expedite services for the target population by providing training and education around the risks of HIV and substance misuse and provide linkages to service provision for those with HIV. Damien Center will conduct targeted community outreach and will adapt three evidence-based interventions/best practices to effectively engage hard to reach individuals. A targeted marketing/digital media campaign and personal outreach in settings frequented by the target population will help identify potential candidates for the program. Interventions will include Think Twice, Motivational Interviewing, and Peer Recovery Coaching. Clients enrolled in the program will be encouraged to participate in three additional programs if assessments identify the need, including HIV/viral hepatitis testing and risk assessment, CLEAR counseling, and substance use counseling. Damien Center will enroll a minimum of 250 individuals into the program (30 in Year 1, 55 per year in Years 2-5). Goals and objectives include: Goal 1: Expand the number of Black and LatinX MSM exposed to substance misuse and HIV education (measurable objectives include identification of community outreach partners, individuals reached through outreach events, social media impressions); Goal 2: Increase the number of Black and LatinX MSM screened for HIV and viral hepatitis (objectives include number of HIV and viral hepatitis screenings completed and education provided to other healthcare providers on the importance of screening); Goal 3: Increase the number of Black and LatinX MSM linked to prevention/substance interventions (measured through enrollment and engagement in Peer Recovery Support Services, engagement with addictions counselor, and enrollment in Think Twice); Goal 4: Increase the number of Black and LatinX MSM linked to and retained in PrEP services (objectives include linkage to and retention in PrEP); and Goal 5: Increase the number of HIV positive Black and LatinX individuals retained in medical care and reaching viral suppression (objectives include linkage to and retention in medical services and level of viral suppression).
