Population to be served: The Ali Forney Center, (AFC) the largest provider of comprehensive services and housing for homeless lesbian, gay, bisexual, transgender and questioning (LGBTQ) youth in the nation, will engage those most in need of substance abuse and HIV prevention navigation programs: 16-24 year old homeless or unstably housed youth who identify as LGBTQ and are predominately people of color from across New York City. Strategies/Interventions: To address the high rates of substance use disorders and HIV infections among homeless LGBTQ youth of color, the program will provide prevention navigation at our 24/7 Drop-In Center in Harlem, New York City. The program will utilize the Intensive Outpatient Treatment Model and HIV Prevention Evidence-Based Interventions to maximize impact. Project Goals: The overarching goal of the proposed program is to provide LGBTQ-competent, specialized substance abuse and HIV prevention services to reduce disparities in access, service use, and outcomes by utilizing the Prevention Navigator model of care. This proposed program will fund the expansion of AFC’s mental health and physical health departments, by hiring a dedicated Transgender Services Coordinator and a Transgender Substance Abuse Counselor as Community Health Workers. Measurable Objectives: 1. Identify and connect 100 homeless 16-24 year old bisexual/gay/transgender and gender non-conforming racial/ethnic minority homeless youth with SUD who are at high risk for HIV, to the Transgender Services Coordinator for substance abuse and/or HIV risk reduction services (500 over project lifetime). 2. Retain 80% in care for the 6-month follow-up and discharge assessments. 3: Achieve a 50% increase in the 6-month change report in the number of youth abstaining from substance abuse and HIV risk behavior. 4: Achieve a 100% Viral Suppression rate for youth who have been referred to treatment services to address their SUD and HIV, and who are also actively engaged in mental and physical healthcare at AFC’s on-site clinic.
