BronxCare Health System’s Department of Pediatrics, with support from Family Medicine and 4 well-established community-based partners, will implement the Prevention Navigator Program. The purpose of the program is to utilize a peer navigation approach to provide HIV and substance abuse prevention services to racial and ethnic minorities (including LGBTQ persons, YMSM, and victims of sex trafficking of any age) in the south and central Bronx. The population of focus is reflective of the service area population which is largely comprised of racial and ethnic minorities (56.4% Hispanic/ Latino, 43.6% African American/Black, and 35.3% foreign-born). Bronx County has been identified as one of the 48 counties hardest hit by the HIV epidemic. Individuals in the target population face significant economic, cultural/language, and psychosocial challenges which often obscure their attempts to seek and remain engaged in care. The following program goals and objectives will be implemented: Goal 1. Enhance partnerships in the implementation of comprehensive community-based substance misuse and HIV prevention strategies. Obj. 1. By the start of month 4 BronxCare will conduct initial program set-up meetings with community partners. Obj. 2. Conduct outreach and engagement to 750 individuals in Year 1, and 1,000 individuals per year for Years 2-5, directly facilitated by partner sites. Goal 2. Develop and implement public messaging and awareness campaigns on the risk of substance misuse among individuals living with HIV and the importance of seeking care and treatment. Obj. 1. By the start of month 4, implement radio, tv, and print for borough-wide reach. Obj. 2. By the start of month 4, implement social media messaging using a minimum of 3 relevant social media platforms to reach the target population. Goal 3. Identify at-risk individuals to engage them in screening and testing for HIV and viral hepatitis in community- and clinic-based settings. Annually assess and screen a minimum of: Obj. 1. 100 participants for substance misuse. Obj. 2. 100 participants for HIV. Obj. 3. 100 participants for viral hepatitis. Goal 4. Provide peer navigation services to link program participants to care for HIV and substance misuse services. Obj. 1. By month 3, hire 5 peers to conduct peer navigation services over the project period. Obj. 2. Annually link 90% of participants to care. Annually provide follow-up with: Obj. 3. 80% of participants engaged and retained in care. Goal 5. Provide education and training to healthcare providers on the importance of screening for HIV utilizing our EMR screening tools and assuring that clinical care needs are met for individuals infected with HIV. Obj. 1. Annually provide education and training to 50 substance use disorder treatment and other health care providers. The program will serve 350 unduplicated clients annually and 1,750 participants over the project period. Staff will conduct outreach and engagement with 4,750 individuals over the project period.
