Vista Community Clinic’s Prevention Navigator Program will target three high-risk groups: (1) youth in foster care, (2) young adults transitioning from state custody or foster care to independence, and (3) youth on probation in four large cities of north San Diego County. The project will provide intensive education and support services for 160 unduplicated, low-income, minority youth and young adults during four intervention years. The project aims to complete six objectives by the end of the grant term: (1) test 500 youth for viral hepatitis and/or HIV, and link any positives to clinical care; (2) partner with five community agencies to provide education and case management services to 40 high-risk youth per year (160 total) on substance misuse, HIV risk factors, and HIV risk reduction using the Engaging People in Care intervention, a modification of ARTAS; (3) document that 80% of participants can identify at least two risk factors for HIV related to substance use and rate the risk of harm from substance misuse as ‘great’ at intervention post-test; (4) document that 80% of participants who report no alcohol use at baseline will report no alcohol use at the 3-6 month follow up, and 80% of participants who indicate risk for HIV infection at baseline will indicate reduced risk at the 3-6 month follow up; (5) implement 6-8 social media/print campaigns aimed at encouraging youth and young adults to get tested, reduce their risk, and seek care; and (6) provide at least 20 education and training sessions to substance use disorder treatment and other healthcare providers on the importance of screening for HIV and how to link diagnosed patients to clinical care.
