Project REACH OUT "ALCANZAR" proposes to implement comprehensive community-based substance misuse and HIV prevention for 400 LGBTQ and heterosexual Latinx adolescents and adults ages 13-24 in Houston. - Fundacion Latinoamericana de Accion Social (FLAS)/Latin American Foundation for Social Action based in Harris County, Houston, Texas, proposes to implement Project REACH OUT/ALCANZAR, the purpose of which is to implement comprehensive community-based substance misuse and HIV prevention strategies for 80 (annually) and 400 (over five years) unduplicated high-risk LGBTQ and heterosexual Latinx adolescents and adults ages 13-24 and their partners (target population/participants). in Houston, Texas Project objectives include: Objective 1. By November 30, 2020, Prevention Navigator will recruit 80 members of the target population for program participation at partner organizations, community centers, schools, and other places where young Latinx people congregate. Objective 2. By September 30, 2020, the Project Director will schedule outreach activities with at least 5 partner organizations including healthcare providers, schools, the justice system, social services, the faith community, and/or others to reduce the number of new HIV transmissions and viral hepatitis among adolescents and young adults. Objective 3. Within one week of the client's intake, Prevention Navigator will connect at least 40 participants and their partners who are HIV-positive, and not in care, to HIV medical care. Objective 4. Within 2 weeks of the client's intake, Prevention Navigator will connect all 80 participants and their partners to screening, testing, and care, if indicated, for mental illness and/or substance misuse. Objective 5. Starting within 1 week of the client's intake, and continuing throughout the client's participation in the program, Prevention Navigator will provide motivational and practical support to help all 80 participants start treatment, maintain treatment, and become virally suppressed. Objective 6. Within 1 week of client's intake, Prevention Navigator will screen each client for environmental factors that impact their health including nutrition, housing, education, employment, social support networks, family support, and access to health care. Objective 7. By the end of December 2020, the Program Director will have identified the service locations where no less than 50 SUD treatment and other healthcare providers will be trained on issues pertaining to HIV prevention. The chosen Evidence Based Practices (EBP's) for Project REACH OUT/ALCANZAR are Toward No Drugs (TND), El Joven Noble Rites of Passage and RESPECT. Services will include general outreach; LGBTQ and heterosexual Latinx outreach; HIV/Hepatitis C testing; HIV and Hepatitis care management; case management; and prevention navigation services.
