CFHW's Resilient Futures: Trauma-Informed Integrated Care for Children and Families - Clinica Family Health and Wellness proposes Resilient Futures: Trauma-Informed Integrated Care for Children and Families to expand evidence-based trauma services for children and youth from birth to age 18 and their families across Boulder, Broomfield, Adams, and Gilpin Counties, Colorado. CFHW will serve 175 children and youth in Year 1, 210 in Year 2, and 240 annually in Years 3–5, totaling 1,105 over five years. Services will include trauma screening, diagnostic assessment, care management, caregiver support, coordinated referrals, measurement-based care, and evidence-based treatment using Trauma-Focused Cognitive Behavioral Therapy, Alternatives for Families: A Cognitive Behavioral Therapy, Let’s Connect, Risk Reduction through Family Therapy, brief and single-session interventions, and trauma-focused practices for youth experiencing first-episode psychosis. By the end of Year 1, CFHW will serve 75 youth through TF-CBT, AF-CBT, and Let’s Connect, 60 youth and families through brief and single-session interventions, and 40 youth through RRFT while training 12 clinicians. At least 70% of youth completing paired assessments for specialized, brief/single-session, and first-episode psychosis services will demonstrate reduced trauma symptoms. By the end of Year 2, CFHW will serve 20 youth with trauma and first-episode psychosis and complete 90% of eligible screenings.
