Substance Abuse and Mental Health Services Administration (SAMHSA) Assertive Community Treatment (ACT) - Central Kansas Mental Health Center (CKMHC) will expand its existing high-fidelity Assertive Community Treatment (ACT) program by establishing a specialized multidisciplinary ACT team for transitional-age youth ages 18–25 with diagnosed or emerging serious mental illness in Saline, Dickinson, Lincoln, Ottawa, and Ellsworth counties in central Kansas. The five-county service area spans 3,787 square miles. The team will provide psychiatric care, medication management, crisis intervention, integrated treatment for co-occurring substance use disorders, individual therapy, supported employment, housing stabilization, peer support, and coordination with primary care and community partners, primarily in community settings using a shared caseload model. The project will strengthen referral pathways and coordination with criminal justice, housing, and primary care systems to identify and engage young adults during transitions to adulthood. Services will begin in Saline County in Year 1, expand to Dickinson County in Year 2, and reach Lincoln, Ottawa, and Ellsworth counties in Year 3. Enrollment targets are 10 participants in Year 1, 15 in Year 2, 20 in Year 3, and 25 annually in Years 4 and 5. Expected outcomes include improved treatment engagement, housing stability, education and employment participation, and reduced psychiatric hospitalization, crisis service use, and justice system involvement.
