Massachusetts Psychosis Prevention Project (M3P) - Massachusetts Psychosis Prevention Partnership (M3P) is a statewide initiative to identify youth/young adults at clinical high risk for psychosis (CHR-P) and provide evidence-based stepped-care interventions to prevent or delay psychotic disorders. Led by the Dept. of Mental Health and a network of clinical partners, M3P integrates outreach, screening, assessment, and stepped-care services thru a coordinated learning health system. Building on M3P-2022 grant work which established the infrastructure and practices necessary to support a statewide CHR-P learning health system and network of care, the 2026 Massachusetts Psychosis Prevention Partnership (M3P) will expand access to stepped-care CHR-P services for MA youth and young adults, particularly those living in rural MA. Geographic Area and Population: M3P serves Massachusetts statewide through seven CHR-P programs and referral partnerships across healthcare, education, child welfare, and behavioral health systems. The program serves youth and young adults up to age 25 experiencing attenuated psychotic symptoms, functional decline, and co-occurring conditions such as anxiety, depression, trauma, and substance use. Of approximately 957,889 Massachusetts residents ages 15-24, an estimated 4,544 experience CHR-P symptoms annually, yet fewer than 10% currently receive specialized services. Over four years, M3P will screen 2,700 youth and provide comprehensive assessment and treatment services to 560 individuals. Strategies and Interventions: M3P combines statewide outreach and education, psychosis-risk screening (PQ-B), SIPS assessment, coordinated referral pathways, and a trauma-informed, community-based, family-driven, youth-guided stepped-care model. M3P CHR-P stepped care interventions include psychoeducation, CBT for psychosis, Acceptance and Commitment Therapy, Family-Focused Treatment, substance use assessment and intervention, academic and vocational support, peer and family support, psychiatric consultation, and crisis planning. M3P achieves high quality, effective services through standardized measurement-based care assessments, extensive EBP training, on-going consultation and supervision, fidelity monitoring, and continuous quality improvement. Goals and Measurable Objectives: M3P will increase access to CHR-P evidence-based stepped care interventions, strengthen the statewide psychosis prevention workforce, improve functioning and recovery outcomes, and reduce progression to psychotic disorders among Massachusetts youth and young adults. • Train at least 500 clinicians, educators, and community partners annually in CHR-P identification, assessment, referral, and intervention. • Conduct 2,700 psychosis-risk screenings and 560 comprehensive CHR-P assessments over four years. • Expand statewide outreach, referral partnerships, and service coordination with faith organizations, primary care providers, education systems, child welfare, juvenile justice, and First Episode Psychosis/Coordinated Specialty Care programs. • Implement measurement-based CHR-P stepped care across all participating CHR-P programs with defined measures of fidelity and program outcomes. • Promote youth and family participation through advisory councils, peer support, family engagement, and quality improvement activities. • Implement individualized crisis plans for at least 90% of youth receiving stepped-care services.
