The Montgomery County Assisted Outpatient Treatment (AOT) Program aims to reduce the incidence and duration of psychiatric hospitalization, homelessness, incarcerations, and interactions with the criminal justice system while improving the health and social outcomes of people with severe mental illness. Montgomery County's AOT program is a multidisciplinary program working across sectors of the community such as the courts, behavioral health providers, and crisis services to support the evaluation of the psychiatric and social needs of the participants, development and implementation of the treatment plan related to the medical, psychiatric, and social needs of the participant, and the provision of referrals to appropriate case management, medical, and social support services to improve the overall health and social outcomes for the participant.
