Project Name: FPT-988 Project
Population to be Served: Tribal members of the Fort Peck Tribes located in northeastern Montana on the Fort Peck Indian Reservation.
Strategies/Intervention: Volunteer Training and Local Referral Resource Development
Project Goals and Measurable Objectives:
GOAL: Develop a permanent crises management/suicide response program on the Fort Peck Indian Reservation to serve as a referral resource for the 988 Voices of Hope Crises Line.
RA 1 Objective 1: By the end of Year 1, the FPT-988 Project staff will have developed a Crises/Suicide Response plan.
RA2 Objective 1: By the end of project month 2 (60 days from award), the FPT-988 Project will submit a Quality Improvement Plan to SAMHSA for the FPT-988 Project.
RA3 Objective 1: By the end of the project period, the FPT-988 Project will have entered into an MOU/MOA with the 988 Voices of Hope Crises Line.
RA3 Objective 2: By the end of the fourth month of the project period, the FPT-988 Project will contract with 1 behavioral/mental health services provider to serve as a local resource for the 988 Voices of Hope Crises Line.
RA 4 Objective 1: By the end of Year 1, the FPT-988 Project volunteers will attend 1 volunteer orientation in Great Falls, MT, hosted by the 988 Voices of Hope Crises Line.
RA 4 Objective 2: By the end of Year 1, the FPT-988 Project will have developed 1 training module for the 988 Voices of Hope Crises Line to include in their volunteer orientations.
RA 4 Objective 3: By the end of month 4 of the project period, the FPT-988 Project will have purchased an electronic health records system to track services and safety plan implementation and updates for clients.
RA 5 Objective 1: By the end of the Project Period, the FPT-988 Project will have developed a sustainability plan for inclusion in the Strategic Plan.
ORA Objective 1: By the end of the project period, the FPT-988 Project will develop a Community Referral Map (that will include traditional activities) for public dissemination.
ORA Objective 2: By the end of the project period, the FPT-988 Project will develop a suicide response team comprised of volunteers from communities on the Reservation to operate the project.

Project Summary: We will develop a permanent program to serve as a referral resource for the 988 Voices of Hope Crises Line in Great Falls, MT. The Fort Peck Tribes have purchased a building in Wolf Point, Montana that is located near the main section of town at 266 Benton Street. This building will be dedicated to operating the Tribes Crises Management/Suicide Call Center, (this name is a working title for ease of discussion until an official name is chosen).
