The purpose of the proposed project is to reduce the co-occurring epidemics of HIV, Hepatitis, and mental health disorders through accessible, evidence-based, culturally appropriate mental health and co-occurring disorders treatment that is integrated with HIV primary care and prevention services. CrescentCare proposes to expand and enhance trauma-informed SUD and/or COD treatment coupled with strength-based case management, behavioral health, HIV and viral hepatitis services for high risk populations, with a focus on individuals, ages 18 and over, of racial and ethnic minorities with a mental health disorder or COD living with or at risk for HIV and/or hepatitis. The project will serve Orleans Parish in Louisiana (encompassing the City of New Orleans) which is one of the localities hardest hit by the HIV epidemic and which qualifies the proposal for the priority points.
The population of focus is individuals, ages 18 and over, of racial and ethnic minorities with a mental health disorder or co-occurring disorder (COD) living with or at risk for HIV and/or hepatitis (HCV) in the greater New Orleans area. These at-risk populations are disproportionately impacted by HIV and HCV, particularly among the African American community in New Orleans.
Goals for the project are to: Increase engagement in care and retention in care for high risk populations living with HIV; Reduce the negative impact of behavioral health problems and substance use through increased access to and retention in treatment for behavioral health and substance use conditions for individuals in high risk populations living with HIV; Reduce the risk of HIV and HCV for high risk populations through outreach and education services; Reduce new HIV and viral hepatitis infections by increasing HIV and viral hepatitis testing and diagnosis; and Improve service integration between outreach and prevention/education, testing, and linkage to primary and infectious disease care, behavioral health and substance use services, HIV care, and HCV care.
The project will serve 50 individuals per year, totaling 250 over the five-year project period and will provide outreach/testing services to an additional 100 individuals each year.
