Finding Wise Mind - Project Name: Finding Wise Mind. Focus population: Adolescents 12-17 with complex PTSD (CPTSD). Catchment area: Harris County, Texas (HC). The project will provide screening and referral services at county primary care sites to 355 adolescents in areas selected for indicators of polyvictimization, and CPTSD treatment combining TF-CBT, DBT, and EMDR along with co-occurring disorder therapy to 150 adolescents with screens indicating both PTSD & CPTSD. CPTSD involves distinct symptom clusters (emotional dysregulation, negative self-concept, disturbance in interpersonal relationships) associated with polyvictimization. Black, Latinx, and LGBTQ youth are more likely to experience polyvictimization, and less likely to access appropriate tx. Goals are 1. Expand access to comprehensive PTSD and CPTSD tx for adolescents by implementing screening and referral at HC Public Health (HCPH) primary care sites in areas with clear indicators of risk for repeated, ongoing exposure to trauma, and by providing CPTSD treatment for youth whose payment resources are inadequate to support CPTSD treatment. Objective 1.1. By Sept 29, 2022, screen & refer 55 adolescents for PTSD/CPTSD at two HCPH sites in high polyvictimization areas of HC. Objective 1.2 By September 29, 2023, 2024, 2025, and by June 2026, screen & refer at least 75 adolescents annually at these sites (=355). Objective 1.3. Comprehensively treat a total of 150 adolescents by September 2026 (=30/yr). Objective 1.4. By September 2026, demonstrate clinically significant improvement for 80%. 2. Improve the quality of services by integrating evidence-supported, staged techniques for addressing the interference of dissociative symptoms in the trauma treatment process. Objective 2.1. By January 2022, obtain advanced dissociation-adapted TF-CBT training and consultation for at least one outpatient therapist and at least one residential therapist. Obj 2.2. By June 2022, begin integrating adaptations into existing CPTSD treatment protocols. Obj 2.3. By September 29, 2022, offer adaptations to all adolescents in C PTSD treatment identified with tx-interfering dissociative symptoms. Obj 2.4. By September 29, 2023-2026, provide dissociation adaptations to a minimum of 8 youth per year, for a total of 37 (including those from the first year) by September 29, 2026. Obj 2.5. By September 29, 2026 demonstrate clinically significant improvement for 95% receiving adaptations. 3. Sustain expanded access and enhanced tx beyond the life of the project by achieving and demonstrating its success to third party payers, and maintaining an ongoing internal training infrastructure to minimize loss due to staff turnover. Objective 3.1. By December 2022, generate and deliver online monthly reports, designed for referral sources, payers, families, and other stakeholders, on clinically significant, documented improvements in outcomes relevant to their aims. Obj 3.2. By October 2023, deliver one outcomes presentation to policymakers concerned with child abuse, health disparities, and adolescent mental health. Obj 3.3. By December 2024, certify two additional therapists as TF-CBT trainers with dissociation focus. Obj 3.4. By Sept. 29, 2025, deliver two additional presentations to potential funding sources. Obj 3.5. By June 2026, secure funding for sustaining key services.
