CNS Healthcare Zero Suicide Initiative: to expand agency standard of care for people at risk of suicide and to create a collaborative of suicide-informed communities - CNS Healthcare is seeking to implement the Zero Suicide model to provide evidence-based prevention and intervention services throughout our healthcare organization to individuals who are 25 years of age or older who are at risk for suicide. CNS provides services to over 5,400 individuals in 9 locations, including five outpatient clinics, two clubhouses, and two drop-in centers as well as a co-located Federally Qualified Health Center (FQHC). All strategies, interventions, goals, and measurable objectives meet criteria for, and include, all seven essential elements of the Zero Suicide model (Lead, Train, Identify, Engage, Great, Transition, Improve). Additionally, the proposed project will expand the capacity of CNS for outreach, engagement, and referrals to adults who are at risk of suicide by developing a suicide-informed community, through building collaborative coalitions with, and providing training for, community partners. This project proposes to serve 3,800 unduplicated individuals over the five-year project period.
