Mountain Comprehensive Care Center (MCCC) will implement the Zero Suicide Initiative to raise awareness of suicide, establish referral processes, and improve the care and outcomes of adults 25 years of age or older who are at risk for suicide. The service area will be Floyd, Johnson, Magoffin, Martin and Pike counties. The project will be implemented across the MCCC health system and will train community agencies on suicide awareness, prevention, and response.
Population: MCCC will target adults 25 years of age or older who are at risk for suicide including such subpopulations as veterans, First Responders and persons with SMI. The service area experiences high rates of poverty, unemployment and significant behavioral healthcare disparities including having a suicide rate 19% higher than the U.S. Research shows a correlation between high-dosage opioid prescriptions and suicide risk making adults in the region more susceptible to suicidal thoughts and actions given the high rates of addiction in this rural area.
Interventions: All adults receiving care will be screened for suicidal thoughts and behaviors using the Columbia-Suicide Severity Rating Scale with those who screen positive referred for a full risk assessment with a trained Clinician and development of a Person-Centered Plan in line with their diagnosis and level of need. Using the least restrictive environment, clients will receive treatment to address suicidal thoughts/behaviors directly using such EBPs as CBT for Suicide Prevention, Dialectical Behavior Therapy, Collaborative Assessment & Management of Suicidality, MI for Suicidal Ideation and trauma-informed care as well as treatment for other mental health or co-occurring SUD issues and wraparound recovery services, as appropriate.
Goals & Objectives: Over the 5-year project, MCCC will screen 560 adults in Year 1 and 1,000 annually in Years 2-5 (4,560), and will provide evidence-based services to 115 adults in Year 1 and 200 annually in Years 2-5 (915). Goals for adults at risk for suicide are to: Improve health by conducting trainings to increase awareness, establish referral processes, and provide evidence-based screening/treatment; Improve stability by supporting continuity of care through access to recovery support; and Improve project implementation/evaluation by conducting CQI. Objectives to be completed by the end of each year are: 1) train and/or provide access for 500 agency staff (300, Yr. 1) on Zero Suicide; 2) conduct 5 community trainings in MH-related practices; 3) screen 1,000 adults (560, Yr. 1) for mental health need; 4) provide evidence-based MH-related services to 200 adults (115, Yr. 1) with 60% improving their MH functioning; 5) provide and/or coordinate housing resources so that 60% (homeless/precariously housed) will improve their status; 6) provide and/or coordinate recovery supports so that 60% will participate in some type of recovery and/or peer support service; 7) coordinate access so that 50% will improve their education and/or employment status, and/or enrollment in disability and/or public benefits; 8) achieve a 6-month follow-up rate of 80%; and 8) conduct quarterly Advisory Council meetings to monitor implementation, referrals, treatment, and goals, objectives, and outcomes.
