Riverside San-Bernardino County Indian Health, Inc. (RSBCIHI) is the largest tribal health care organization in California. Our Zero Suicide project will help us fill six (6) gaps in our current suicide care management framework across all seven (7) of our health centers. Over the five (5) year project, we will screen 25,243 people for depression, suicide ideation, and other risk factors. We estimate that 985 individuals at risk for suicide will receive services through this grant.
Our catchment area covers Riverside and San Bernardino Counties and nine (9) federally-recognized Indian reservations. This area is home to 49,665 American Indian/Alaska Native (AI/AN) adults age 25 years or older. Last year, we screened 4,367 unduplicated adult patients for depression, suicide ideation, and other risk factors. Of these patients, 1,114 (25.5%) screened positive on the PHQ-9 for one (1) or more depressive symptoms and 480 (11 %) screened positive for clinically significant depression. Also, 173 (3.9%) screened positive for suicide ideation. These annual rates are consistent with the data we have collected over the last six (6) years.
Our Zero Suicide project has three (3) goals: 1) Increase our capacity to implement all elements of the Zero Suicide framework in our health centers; 2) Deliver the most appropriate treatment and support to patients at risk for suicide; and 3) Measure our performance to determine whether we are achieving our goals, objectives, and outcomes. To accomplish these goals, we will:
• Expand our screening program to our new Santa Rosa Health Center and our dental and eye care departments to screen an additional 682 unduplicated patients each year.
• Develop protocols to standardize our Suicide Care Management Plan. Currently, each of our behavioral health clinicians uses their own methods for monitoring patients at risk for suicide.
• Deliver weekly training and clinical supervision to five (5) pre-licensed clinical interns from Loma Linda University. Together, these interns will deliver outpatient mental health services to 68 patients each week across five (5) health centers, including treatment for depression, suicide ideation, and/or other risk factors.
• Create a Clinical Care Manager position who will track, monitor, and coordinate care for all suicidal patients according to the new Suicide Care Management Plan. We expect this staff member to monitor an average of 173 suicidal patients at any given time.
• Train 154 staff members involved in patient care, including behavioral health, medical, dental, and eye care providers, to identify and respond to patients with suicide ideation and implement our Suicide Care Management Plan.
• Deliver screening, assessment, mental health intervention, and case management services in accordance with The Joint Commission standards, the elements of the Zero Suicide framework, the required activities of the grant, and other evidence-based practices.
• Organize a Zero Suicide Advisory Committee involving members of nine (9) communities.
