One Stop Wellness of the Finger Lakes - The One Stop Wellness of the Finger Lakes through of the Finger Lakes Area Counseling and Recovery Agency (FLACRA) will establish a Certified Community Behavioral Health Clinic, with sites in two rural counties in the Finger Lakes Region of Upstate New York; Yates and Ontario. The population(s) of focus for this project is adults, youth and children with a SMI, SED, SUD or COD, primarily low-income/Medicaid recipients. The project will provide comprehensive CCBHC services directly and through Designated Collaborating Organizations.
Yates and Ontario counties are designated as Medically Underserved and Health Provider Shortage Areas for Mental Health. The rural nature of this area along with few behavioral health providers, high poverty rates and a limited public transportation system presents challenges for access to care. Special populations within the geographic area include Amish and Mennonite populations in Yates County (estimated to be 20 percent of the county's population) and monolingual Spanish speaking populations primarily in the Ontario County city of Geneva. A total of 1,135 individuals will be served by the Project over the 2 year period, Year 1: 555 and Year 2: 580.
The project goals are to: Fully establish CCBHCs for person with SMI, SED, SUD, COD in the rural communities of Yates County and Ontario County New York. Increase the availability of, and access to, comprehensive CCBHC services for person with SMI, SED, SUD, COD in the rural communities of Yates and Ontario Counties. Provide evidence based, integrated mental health and substance use interventions for treatment and case management services to improve client retention and health outcomes. Provide Peer Support and Recovery Support Services for individuals and their families. Streamline service delivery for the population of focus through the integration and coordination of care across the CCBHC.
FLACRA is a NY State Office of Mental Health (OMH) and Office of Addictions Services and Supports (OASAS) licensed provider of a range of services at multiple sites in the Finger Lakes region of NY, including outpatient clinics, crisis overdose response, stabilization, detoxification, residential, recovery support, care management and home and community-based services (HCBS). FLACRA's existing services include most of the CCBHC required services and will be built upon to extend this array to additional populations (SMI, SED, children under age 12). FLACRA is innovative and the agency lead for Finger Lakes and Southern Tier Behavioral Health Care Collaborative (FLST BHCC) and an active, founding member of the Finger Lakes IPA (FLIPA).
Services will be grounded in person-centered planning approaches, attention to the cultural norms and beliefs, and trauma-informed practices. Evidence-based practices and approaches will be used as appropriate to each individual, including Motivational Interviewing, Cognitive Behavioral Therapy, Rational Emotive Behavior Therapy, Solution Focused Therapy, Matrix Model, Seeking Safety, Living in Balance and CRAFT. Certified Recovery Peer Advocates (CRPA) will be embedded in outpatient services lending additional support to each individual and their family.
