The Center for Human Development (CHD) proposes to use a team-based approach to provide outpatient behavioral health services to adults and families with serious mental illness (SMI), serious emotional disturbance (SED), substance use and co-occurring disorders (SUDs/CODs). The CCBHC project will target vulnerable populations including African-Americans, Latinos, LGBTQ youth, and veterans living in Hampden, Hampshire, and Franklin Counties in Western Massachusetts. By the end of the two-year grant period, CHD will serve 700 unduplicated persons. CHD will implement the proposed CCBHC-E project by building on the framework created during the 2018 CCBHC grant, which deployed Community Health Teams (CHTs) at four of CHD’s eight outpatient clinics in two counties (Hampden and Franklin). Each CHT consists of a licensed clinician, two case managers, a half-time RN/Health Educator, two recovery coaches and a quarter-time prescriber. The proposed 2020 CCBHC-E project will continue these teams and extend the CHTs to two additional outpatient clinics—one in Hampden County and one in Hampshire County—bringing the total to six clinics and three of the four counties in Western MA. The proposed project will also expand primary care services dedicated to serving CCBHC-E consumers in the three clinics in Hampden County, a HPSA-designated area. In the two-year grant period, the proposed project will: 1) reduce the incidence of opioid-related overdoses and deaths through outreach and providing MAT and other recovery-oriented support services; 2) improve health outcomes for persons served who have SMI, SED, and SUDs/CODs by expanding primary care services and monitoring health indicators; 3) improve outcomes by standardizing the screening and support of social determinants of health vulnerabilities with the use of the Arizona Self-Sufficiency Matrix; 4) increase CHD’s clinic capacity to address and promote recovery for both mental health and substance use disorders. The project will serve 350 individuals in year one; and 350 individuals in year two for a total of 700 unduplicated persons.
