Family Service League (FSL), proposes to increase access to, as well as improve the quality of person and family centered and integrated community mental health and substance use disorder treatment services, through the expansion of CCBHCs. Population of Focus: Adults, families and veterans with diagnoses of SMI/SUD/OUD/COD and children 5+ with SED. Geographic Catchment Area: Suffolk County, NY. Per US Census, 2018 and Vintage 2019 estimates, the demographic characteristics are: Population: 1,481,5093; Veterans – 64K. Age/Gender: 5.4% under 5 years; 21.1% under 18 years; 16.9% over 65 years; 50.8% female; and 49.2% male. Race/Ethnicity: White – 84.4%; Black – 8.7%; Native American – 0.6%; Asian – 4.2%; and Hispanic/Latino – 19.8% (2 or more races counted twice). Suffolk has long been described as ground zero for the heroin/opiate epidemic (2017, Long Island Interventions). The region has the highest percentage of patients (21-64) with schizophrenia & other psychotic disorders (41%) and the 2nd highest percentage with bipolar/related disorders (22%). The region has the highest percentage of patients under 21 with depressive disorders (26%). (NYSOMH RNA, 2016) Goal: Provide comprehensive, 24/7/365 access to a full-continuum of coordinated, person and family centered, as well as integrated community-based mental health and substance use disorder services, to include: crisis intervention services for individuals with SMI/SUD/OUD/SED/COD. Objectives: Objective I: By December 30, 2020, FSL’s Behavioral Health Program will provide a comprehensive, coordinated and fully integrated (primary care/behavioral health) collection of services to ensure full compliance with CCHBC certification requirements. Objective II: By December 30, 2020, FSL will provide 100% coordinated, integrated and comprehensive 24/7/365 access to person-centered behavioral health crisis services in the Riverhead BHC per FOA requirements. Objective III: By December 30, 2020, FSL will increase professional, peer and supervisory staffing capacity by 85% at the Riverhead Clinic to ensure safe, same day availability. Objective IV: By December 30, 2020, 100% of FSL staff employed in the three East End crisis service programs will deliver person-centered, CLAS. Objective V: By December 30, 2020, 30%; by December 30, 2021, 60%; and by December 30, 2022, 100% of Riverhead ’s prescribing staff will provide MAT. Objective VI: By December 30, 2020, 100% of FSL staff will provide recovery focused and patient centered care during the use of MAT. Objective VII: By October 30, 2020, FSL will develop a comprehensive behavioral health disparities impact statement consistent with FOA requirements. Objective VIII. By December 30, 2020, FSL will provide outpatient primary care screening/monitoring of key health indicators/health risk; screenings/assessments for HIV/AIDS, viral Hepatitis A, B and C, and vaccinations to 100% of new clients. Objective IX. By December 30, 2020, FSL will provide comprehensive outpatient mental health and substance use services, consistent with FOA conditions and requirements, to 100% of new clients.
FSL proposes to serve 600 clients (450 adults &150 children), annually; and 1200 total.
