PROJECT SUMMARY: LifePath Systems will implement the Collin County CCBHC Expansion Project to serve 2,500 children and adults with serious mental illness (SMI), severe emotional disturbance (SED), and/or substance use disorders (SUD). The project will expand access to care coordination and tele-psychiatry services, as well as expand workforce training efforts to significantly enhance the quality of services and supports provided to this population.
POPULATION & NEED: Research indicates there are an estimated 42,000 Collin County adults with an SMI and over 29,000 children with an emotional disturbance or addictive disorder, with nearly 7,000 children suffering from a SED or severe addictive disorder. Collin County is projected to triple its population and reach 2.4 million residents by 2050. With rapid growth comes dramatic shortages in resources to serve people with mental illness and/or substance use disorders, especially those with serious, severe, or co-occurring disorders. Through the Collin County CCBHC Expansion Project, LifePath Systems will serve low-income and uninsured residents of Collin County, Texas, who have an SMI, including schizophrenia, bipolar disorder, major depression, and those with a sole diagnosis of substance abuse or those with a dual diagnosis of mental illness plus either a substance use disorder or a developmental disability.
STRATEGIES: The project will help improve services for this population by establishing a Care Coordination Team to help individuals access physical healthcare and treatment for SMI and/or SUD, while also assisting individuals with accessing, locating, and securing housing, employment and transportation to ensure the well-being of each person served. The project will also address the need for timely access to psychiatric services by expanding its current tele-psychiatry contract to offer more availability, expanding to three tele-psychiatry locations where individuals can access services, and equipping Care Coordinators with laptops and tablets that can be used by individuals for tele-psychiatry during in-home visits. Finally, the project will expand and enhance its workforce training in evidence-based practices, cultural and linguistic appropriate services, trauma-informed care, person-centered services, military/veteran culture, and co-occurring disorders. This will improve the quality and competencies of workforce members who provide critical services and supports for people with SMI and/or SUD.
GOALS, OBJECTIVES & PEOPLE SERVED: The goals of this project are to: (1) expand care coordination services; (2) improve timely access to psychiatric services; and (3) improve the skills and expertise of the workforce. Annually, the objectives are to: (1.1) establish a care coordination team to improve timely access and quality of care for 500 individuals; (2.1) expand tele-psychiatry service contract to serve 750 individuals; and (3.1) expand online and in-person trainings to improve competencies of 250 workforce members. Over the lifetime of the project, the total number of unduplicated people served will include: 1,000 by care coordination; 1,200 by tele-psychiatry; and 250 by workforce training.
