Network180 CCBHC Project - Based in Grand Rapids, Michigan, the Network180 CCBHC Project provides expanded access to mental health and substance use treatment to all who are in need. The project funds 10 clinicians to expand outpatient therapy, 6 crisis workers to provide mobile crisis intervention, and 2 assessment staff to ensure quick access to services. Primary care services delivered by Cherry Health will be integrated with mental health services to ensure wholistic recovery.
The population to be served includes people of all ages who present at Network180 with a Mental Illness (MI), Substance Use Disorder (SUD), or Co-occurring Disorder (COD), including veterans. Kent County has a population of over 650,000 and is 82% white, 11% African American, 11% Hispanic, 3% multiple races, 1% American Indian, and <1% other. Veterans are about 5% of the population. Adults with a serious mental illness and children with a serious emotional disturbance make up 4% of the population, and 5% have a substance use disorder.
For the approximately 7% of the Kent population with no health insurance, outpatient services are prohibitively expensive. There is little outpatient availability even for the 15% with Medicaid, as provider reimbursement rates are about half of what it costs to provide the service. Without proper treatment, the symptoms of those struggling with mental illness or substance use can deteriorate into dangerous crisis and there are inadequate crisis mental health services to intervene. Consequently, many of those in crisis end up at emergency rooms or the jail.
This project will significantly expand access to outpatient services and crisis mental health care. Network180 will serve 100 new outpatient clients within the first 6 months with therapy and medication management for mental health and substance use. Mobile crisis response teams will be available 24/7/365, providing a team to assist the client in crisis whether in their home, work, school, or elsewhere in the community.
Network180 is partnering with Cherry Health, a Federally Qualified Health Center, to provide comprehensive primary care and physical health services. Cherry Health will provide telehealth Suboxone treatment for opioid use disorders. Network180 clinical staff will provide systematic screening for health risk factors, screening and counseling regarding BMI and problematic substance use, education and medication for smoking cessation, and lab services including screening for HIV and Hepatitis A, B, and C.
Network180 is partnering with Arbor Circle, Pine Rest, InterAct, and Hope Network to offer targeted case management, Assertive Community Treatment, psychiatric rehabilitation, social support services, and community recovery supports. Partnerships with the Lakeshore Regional Entity and the local VA will assist veterans by providing staff training and system navigation.
The CCBHC will serve 2000 unduplicated individuals annually for a total of 4000 served during the life of the project.
