Achieve Behavioral Health, a successful CCBHC since July 2017, and a service provider for over 35 years, is submitting a CCBHC expansion grant application to leverage its work as a CCBHC to implement an Assertive Community Treatment (ACT) and Integrated Clubhouse and Clinic (ICC) service that will close the gaps that exist for services to adults and adolescents with serious mental illness (SMI), co-occurring disorders (COD) and for whom additional supports and easier access to care is needed in New York Rockland and Orange Counties. The ACT services are targeted for high-risk/high-need clients unable to receive adequate care through our clinic or Intensive outpatient program alone. The ACT services offer a full range of coordinated clinical and support services at-home or community to maximize positive intervention. The Integrated Clubhouse and Clinic (ICC) are targeted for ACT clients, clients with SMI, SUD or any clients needing more engagement, service and support, and easier access to care. The ICC services offer peer-led accessible, wide-ranging supports and skills with integrated clinical services. Clinical services offered as integrated into the clubhouse are Behavioral Health and Substance Use Disorders (SUD) (individual and group, scheduled and drop-ins) and a full service Primary Care service to maximize physical health in this vulnerable and underserved population. Achieve will offer ACT and Clubhouse services to 175 individuals within the first year of the grant (50 ACT, 125 clubhouse), with 440 (115 ACT, 325 Clubhouse) being served throughout the lifetime of the project. Through the expansion grant Achieve will offer services that augment current CCBHC integrated care to further support the identified vulnerable population in achieving these measurable goals: effective symptom management, reduction in substance abuse including opioids, improved access to and engagement with medical providers, reduction in inpatient stays, and access to and retention in housing. The population of these two counties are of the most rapidly growing in the NY and includes a high density of the historically underserved Ultra-orthodox and Hasidic Jews.
