The Ottawa System of Care (SOC) Grant will serve youth aged birth to 21 and their families living in Ottawa County, with an emphasis on those living in poverty, those experiencing or at risk of serious emotional disturbance and/or substance use, or those under-served by current community services. Ottawa SOC will focus on education, dissemination, implementation and full-scale adoption of an integrated, culturally competent, trauma-informed, evidence-based system of care for youth and families. Child-serving systems and agencies will work collaboratively to develop a comprehensive, coordinated and responsive System of Care. The SOC will be family-driven, youth guided, trauma-informed and culturally and linguistically proficient at the system, practice and service levels. Funds will be used to develop the infrastructure for this county-wide approach, provide training and technical assistance to behavioral health providers, juvenile justice personnel, school staff, and youth and their families. Services for the target population will be enhanced through expansion of mobile response services, targeted outreach to schools, and increased services to uninsured families. The goals for this SOC grant are to 1) Develop and implement the Ottawa SOC structure to improve community-wide approach to at-risk youth, 2) Promote and sustain family engagement and youth-guided activities across the SOC, 3) Increase access to care for at-risk youth by implementing Zero Suicide approach in targeted schools to provide support and education to schools for addressing suicide concerns; 4) Increase access to care for at-risk youth and reduce gaps in care by developing and conducting an anti-stigma campaign, and 5) Increase access for at-risk youth and reduce gaps in care by expanding mobile response team capacity. Over the course of this project, 205 youth will be served (25 in year 1; 45 in year 2; 60 in year 3; and 75 in year 4) through these service enhancements.
