Vaya Health and Youth Villages will partner to expand system of care in 5 identified western North Carolina counties building upon the Family and Youth Partner model. Through extensive cross sector collaboration in the identified counties, infrastructure will be developed to better serve families and youth who are identified as high risk for out of home placement or with severe emotional disturbance. The model will support stabilizing families while addressing trauma and social determinants of health using evidence based practices to include but not limited to: trauma informed care, screening, assessments, and trainings. Through the support of Family and Youth Partners, families and youth will learn advocacy skills, build resiliency, and identify natural supports.
