"Growing the Caring Community" is Elkhart County Indiana's proposal for system of care (SOC) expansion. Central to this project is its system of care, The SOURCE. It will further its growth by nurturing a "Community Asset Care Coordination" model, employing Asset Based Community Development (ABCD) methods to bring the broader community together with agency partners in promotion of expanded supports and services for children's mental health. In addition to SOC infrastructure enhancement, we will increase services through this project. In the context of a broad continuum of care within the SOC, the project will serve children/youth age birth-21 with, or at risk for, significant mental health challenges (SED). Research and evaluation suggest three focal points for our expansion: un- and under insured children/youth or youth with significant mental health challenges at risk for juvenile justice referral; services promoting early childhood social emotional development; and minority children and youth across the SOC who experience disparities or disproportionality. An integral part of this expansion will include parent/caregiver/youth empowerment strategies as well as other SOC infrastructure enhancements in policy development, governance, social marketing and sustainability efforts. The project will be connected to Indiana's SOC activities. Our goals include: (1) Increasing capacity of The SOURCE to provide access to services and supports that promote the emotional well-being of children/youth through an expanded care coordination model built on an ABCD approach; (2) Broadening our early childhood SOC by focusing on successful kindergarten transition of children with identified social emotional challenges from early childhood sites piloting Conscious Discipline and early childhood mental health consultation; and (3) Reducing behavioral health disparities or disproportionality within the SOC. Specific objectives include: increasing access to behavioral health treatment for un- and under insured youth; creating new linkages between community assets and behavioral health services/supports; increasing positive behavioral health outcomes for referred youth; identifying and effectively addressing treatment needs of youth with mental health challenges referred to the juvenile justice system; growing advocacy opportunities for family and youth; training partners in evidence based practices; increasing SOC outreach and resource activities; increasing social emotional skills in young children; increasing early childhood staff capacity in social emotional development; increasing successful kindergarten transitions; increasing the number of minority youth with behavioral health concerns who are successfully diverted from the juvenile justice system; reducing disparities in treatment outcomes for minority youth; increasing successful kindergarten transition for minority children. These will be measured by indicators of certain clinical, consumer satisfaction, system and program outcomes and the required GPRAs.
