For over two decades, the Hawaii State Child and Adolescent Mental Health Division (CAMHD) has followed the System of Care (SOC) model and provided evidence-based mental health services to youth with Severe Emotional and Behavioral Disturbances. The proposed project is designed to extend SOC principles statewide by transforming data from various sources of evidence to wisdom at the youth, provider, and system level. Although progress toward a fully-implemented SOC has been made in many areas, there are still many challenges which this project would address: (a) utilizing data from a rich array of sources to guide clinical decision-making at the youth, provider and system levels, (b) coordinating efforts efficiently across child-serving system agencies, (c) expanding access to and improving our wraparound program, and (d) providing in-home services that are maximally effective at strengthening families and preventing out-of-home (OOH) placement. The CAMHD has established a strong infrastructure for interagency collaboration at the state level by establishing a group (i.e., the Hawaii Interagency State Youth Network of Care; HISYNC) that meets monthly and includes broad representation from child-serving agencies and a parent-partner organization. This project will utilize HISYNC members as its Interagency Governing Council and will work to strengthen the group's functioning, visibility, and capacity for joint programming and decision-making. Additionally, CAMHD has been on the forefront of evidence-based practices (EBPs) by offering an array of recognized evidence-based treatments (MST, FFT, MTFC, TFCBT), by promoting the use of modular psychotherapy in in-home services, and by adopting a practice element distillation model to systematically collect data on a range of indicators. Nonetheless, 11% of youth trajectories decline as evidenced by high utilization of restrictive OOH treatment programs. The proposed project would utilize a Knowledge Management (KM) approach and participatory system dynamics modeling to improve trajectories at all levels of the service system, with the ultimate goal of improved youth outcomes (e.g. decreased use of out-of-home programs, decreased lengths of stay, improved vocational and school outcomes). In recent years, the CAMHD has invested considerable resources in improving our data management and electronic case management system that opens up new possibilities for utilizing a richer set of real-time data in decision-making at every level - including decisions about programs and policy issues, decisions about referring a specific youth to a particular treatment, and finer-grained decisions about which practice elements to utilize and how to adjust the timing of interventions within a chosen treatment. The proposed project will work to develop, train on, and utilize the KM approach, specifically to reduce OOH placement by improving our wraparound program and to guide clinical decisions in IIH therapy.
