The Swinomish Native Connections Grant (SNC) seeks to grow a Pacific Northwest tribal systems transformation to provide comprehensive, coordinated and culturally grounded care to all Swinomish children and youth (10-24) with complex needs and their families. The primary goal is to expand the Swinomish Health Programs to fully integrate and coordinate behavioral health services with substance abuse and chemical dependency and child welfare programs and agencies servicing the 1,000 Swinomish tribal members on the reservation and in the surrounding urban areas of the Pacific Northwest. Utilizing a local indigenous theory of change, the SNC aims to increase resilience and recovery for Swinomish children, youth and families by: 1) address unmet mental health needs for our youth; 2) culturally tailor our evidence-based clinical services and preventative practices; 3) reduce trauma-related risk factors; 4) promote healthy, culturally rooted and multilevel preventive interventions; 5) culturally enhance existing EPBs/clinical services and engage all Swinomish youth between the ages of 10-24 in culturally-grounded interventions that build multilevel protective factors and increase reasons for life and reduce risk for suicide and substance misuse; 6) creating sustainable systems and service structures that can continue to grow and heal from within; and 7) assessing outcomes from the expansion utilizing an indigenous evaluation framework to gather data demonstrating the efficacy of the infrastructure changes and clinical-cultural services expansion for achieving behavioral health outcomes with 400 Swinomish children and youth. The Swinomish Indian Tribal Community has a vision of addressing the impacts of trauma, healing the past and moving forward towards a hopeful future for our children and our Nation. Grown from roots of resilience, our systems, like our children and our Nation will grow strong like the cedar.
