The Healthy Transitions program is a culturally responsive, youth/young adult driven and guided program, involving family and community members to provide continuity of care and support between child and adult serving systems. The goal of our project is to improve access to treatment and support services for youth and young adults, ages 16-25, who have a serious emotional disturbance or a serious mental illness. This includes those will intellectual developmental disabilities, substance abuse disorders and co-occurring disorders. Wraparound services provided will be developmentally appropriate, culturally and linguistically competent services and supports. The targeted population is both Native Americans and non-Native Americans who reside in a rural setting on the Olympic Peninsula in Washington State, both on and off several reservations. Our county is one of the top six Washington State counties that is of a lower socioeconomic population with limited resources within their communities statewide. Our goal is to screen approximately 300 youth and young adults ages 16-21 and provide wrap-around mental health treatment and support services to approximately 200 of our targeted population for the life of the project. During year 1 we will serve 24 clients (within 4 months of receiving the award), and increase that number each year by 44 clients for the remaining 4 years, in totally serving 100 clients at a minimum. It is expected this program will improve emotional and behavioral health functioning so that this population of youth and young adults can maximize their potential to assume adult roles and responsibilities and lead full and productive lives. This will be accomplished by increasing public awareness and cross-system provider trainings, screening and detection, outreach and engagement, referrals to treatment, coordination of care, and evidence-informed treatment. We will improve cross-system collaboration, service capacity, and expertise related to the focus population through infrastructure and organizational changes at the Tribal level. Training to partners, staff, youth/young adults, and community members will be offered throughout the grant on the National CLAS standards, Suicide Prevention, First Aid Mental Health (Youth and Adults), Wraparound in Indian Country, as well as others that are deemed vital to the implementation and success of this project.
