The purpose of this program is to improve access to treatment and support services for youth and young adults, ages 16-25, who have a serious emotional disturbance or a serious mental illness. - Healthy Transitions will use evidence-based/informed approaches - High Fidelity Wraparound and Transition to Independence Process (TIP) - to reach and serve youth and young adults (YYA), aged 16-25, that have serious mental health illness (SMI) or serious emotional disturbance (SED) and have "fallen through the cracks" utilizing culturally competent, low barrier engagement and extensive outreach in two diverse local laboratory communities. Almost 20% of YYA have a diagnosed mental illness. Despite this, more than 50% of those with diagnosable illness do not receive treatment. In other words, more than 50% fall through the cracks. The goals of Healthy Transitions are: to increase YYA voice and create a strong young adult peer support network; to create community, adult and child state agency cross-collaborations to identify unconnected YYA; to implement a YYA-informed public awareness/social media campaign; to create YYA-informed organizational and systems standards of practice; to implement evidence-based/informed approaches to better serve YYA who have not been successfully reached; and to launch outreach/engagement strategies tailored to the cultural, linguistic and sexual orientation needs of the communities served. Success across these six goals will produce a sustainable system of outreach, engagement, and service delivery to improve lifelong trajectories to more than 7,000 unconnected YYA in the targeted communities over the lifetime of the grant. Healthy Transitions will create a services infrastructure to, at the state level, eliminate barriers for unconnected YYA, to expand YYA-serving practices throughout the service system, and to facilitate blended and braided funding mechanisms to effectively serve YYA. At the local level Healthy Transitions will identify unconnected YYA, connect them to needed services, and facilitate delivery of evidence-informed services to address mental health and related needs. In addition to the two evidence-based/informed approaches - Transition to Independence Process (TIP) and High Fidelity Wraparound - Healthy Transitions will draw upon other evidence-informed practices to provide YYA services/supports, including Achieve My Plan, Motivational Interviewing, SBIRT (Screening, Brief Interventions, Referral to Treatment) and Drop-in Centers, Health Transitions will deliver services in Lowell, an under-resourced, highly diverse immigrant community with 20.9% Latino population and 18.2 Southeast Asian population; and Gloucester, a primarily white community with higher than average demonstrated need in services for YYA. The Massachusetts Department of Mental Health is well poised to implement this Healthy Transitions grant as evidenced by their lessons learned and successful implementation of Now is the Time Healthy Transitions Grant, their long-standing commitment to youth and young adults with serious mental health conditions, and their role a sa national leader in developing services and policies for this at-risk population.
