The District of Columbia's population of focus for the Healthy Transitions: Improving Life Trajectories for Youth and Young Adults with Serious Mental Disorders Program initiative is transition age youth (TAY) and young adults ages 16-25 years old, who have a serious emotional disturbance (SED) or a serious mental illness (SMI). The District's initiative will be called OurTime: Exploration. OurTime: Exploration will focus on District Wards 1 and 6. In the first year of the grant, 40 unduplicated youth will be served; in Years 2-5, it is estimated that 80 unduplicated youth will be served for a total of 360 unduplicated youth that will receive direct treatment services and/or recovery support. The OurTime: Exploration initiative will support the District's Department of Behavioral Health's (DBH) mission to integrate substance use disorder and mental health services and supports and provide the right services and supports at the right time in the right amount. The goals of the initiative are to: increase the self-efficacy and meaningful participation in transition plans of young adults ages 16-25 who have mental health and/or co-occurring substance use disorders; improve and expand treatment recovery and support services and strengthen evidenced-based practices that address all life domains; increase community buy-in and ownership of TAY outcomes through establishment of OurTime: Exploration Transition Implementation Team and the continuation of the TAY Roundtable Takeover to increase community knowledge, support, and responsiveness to TAY; develop and implement community-wide education and messaging to reduce stigma about getting help for mental health and co-occurring SUD. As the lead on the OurTime: Exploration initiative, DBH will leverage and enhance the system of care created through the Substance Abuse and Mental Health Services Administration (SAMHSA) State Adolescent Treatment Enhancement and Dissemination (SAT-ED) and the Now is the Time, Healthy Transitions grants. TAY and families will systematically be screened across multiple settings including schools, juvenile/criminal justice, and child welfare for substance use disorders/co-occurring disorders and trauma. Based on youth/TAY/family needs, providers will deliver a cadre of evidence-based practices including: Transition to Independence Process (TIP), Transition to Independence Process and Assertive Community Treatment (TACT), Peer Support Services, Adolescent Community Reinforcement Approach (A-CRA), Clubhouse model of psychosocial rehabilitation, Cognitive Behavioral Therapy for Psychosis (CBTp), and Motivational Enhancement Therapy/Cognitive Behavioral Therapy.
