The purpose of Tennessee's Healthy Transitions Improving Life Trajectories HT-ILT Initiative is to improve emotional and behavioral health functioning of youth and youth adults YYA ages 16-25 who have a serious mental disorder or co-occurring intellectual disability IDD. Through partnerships with local providers, HT-ILT will increase access to evidence based treatment and recovery support services through outreach, engagement strategies, coordinated public awareness, a team based service delivery model and young adult peer stabilization services. Tennessee Dept of Mental health and Substance Abuse Services TDMHSAS is committed to ensuring that those YYA that are most likely to "fall through the cracks" and least likely to seek help receive the services and supports they need, maximizing their potential to assume adult roles and responsibilities and lead full and productive lives. TDMHSAS, the locus of accountability for HT-ILT, is responsible for oversight of state and local grant related activities and outcomes which are based on the three HT-ILS goals. These are to: 1. Increase and enhance awareness, outreach and engagement efforts for YYA within the focus population; 2. Create, implement, and expand services and supports that are developmentally appropriate, culturally and linguistically competent, and youth and young adult driven, and provide for continuity of care and support between child and adult serving systems for YYA within the focus population in two target communities; and 3. Improve cross-system collaboration, service capacity, and expertise related to YYA within the focus population through infrastructure and organizational change at the state and local levels. TDMHSAS will partner with community mental health providers in an urban and a rural community, Davidson and Greene Counties, to provide team based wraparound and recovery support services, with a goal of serving 30-45 YYA annually and a total of 195 YYA within five years. TDMHSAS will also partner with a family-run organization to provide young adult peer stabilization services within the mobile crisis system in Davidson County and to promote outreach and engagement though coordinated social marketing efforts. As the single state authority for child and adult mental health, TDMHSAS will develop a Memorandum of Understanding to ensure seamless accessibility to mental health services for YYA who are transitioning to adulthood, and will be the core of the State Transition Team STT, which will be responsible for statewide infrastructure activities. The STT will be comprised of various stakeholders across the child and adults serving systems and will include YYA representatives, leveraging the ability of YYA to recommend improvements to the child and adult mental health systems. The coordinated approach of HT-ILT will increase workforce capacity, create a more robust continuum of care for YYA, promote best practices for YYA engagement and service delivery, and improve state and local infrastructure to better serve YYA with serious mental disorders and co-occurring IDD as they transition to adulthood.
