Certified Community Behavioral Health Center - The Center for Health Care Services (CHCS), the local mental health authority for San Antonio/Bexar County, Texas
seeks to expand the availability of integrated physical and behavioral health care, enhance accessibility to care
coordination and continuity care services, and build the infrastructure required for the consistent delivery of
highest quality care to 3,000 adults and children with life altering behavioral and physical health conditions.
Project Name: The Center for Health Care Services’ CCBHC Project
Population to Be Served: Low-income adults and children with serious mental illness, co-occurring substance use
disorders and mental illness, serious emotional disturbance, and chronic physical health illnesses or conditions.
Strategies and Interventions: CHCS will improve the functionality and quality of its integrated care continuum
by: i) expanding care coordination and continuity care functions to support treatment retention and effective
transitions across the care continuum; ii) adopting uniform, system-wide policies and procedures to promote the
consistent provision of high quality care for adults and children; iii) expanding current monitoring and
continuous quality improvement practices to ensure highest level fidelity and impact at the service and
organizational levels; and, iv) using data to support population health management and the delivery of timely,
appropriate, integrated care.
Project Goals and Measurable Objectives: Goal 1: Operate a comprehensive continuum of care that creates access,
stabilizes people in crisis, and provides treatment and recovery supports for adults and children with the most
serious, complex behavioral health disorders. Objective A. Install four Care Coordination Teams (1 Care
Coordinator and 1-2 Peer Support Specialists) at points across the continuum (engagement, treatment, aftercare)
to support 1,500 consumers per year. Objective B: Operate four integrated care clinics that meet CCBHC standards
while treating 1,500 adults and children per year. Goal 2: Supplement or refine the operating processes essential
to CHCS’s full conversion to a CCBHC system. Objective A. Develop and implement uniform, organization-wide
policies and procedures for clinical operations, resulting in a rate of consumer satisfaction that is 75% or
higher. Objective B: Establish, test, refine and operate within an enterprise-wide monitoring and compliance
framework, and use standardized continuous quality improvement processes to ensure outcome achievement at the 85%
level or above. Objective C: Improve data management processes, examining and retooling risk stratification
practices, the use of patient registries, population health management, the consistent identification of unmet
needs, and the use of patient-centered, patient-friendly processes.
Number of People to Be Served Annual and Throughout the Project. 1,500 adults and children per year, 3,000 across
the project period.
