HealthWest CCBHC Expansion Grant Application - HealthWest's CCBHC Expansion Grant project "CCBHC: Increasing Access, Expanding Services and Improving Treatment" will enhance and expand the continuum of care for behavioral healthcare services in Muskegon County. HealthWest's behavioral health services project will address unmet needs in the community by broadening service offerings and providing treatment to underserved populations of Muskegon County. Specifically, HealthWest will emphasize being able to effectively respond to the community's growing substance use disorder treatment needs and the behavioral health needs of youth while addressing gaps in care for individuals in rural communities, veterans, and Hispanic individuals and families.
HealthWest is the Community Mental Health Services Program (CMHSP) of Muskegon County, Michigan. Muskegon County is a diverse county in western Michigan that blends urban and rural settings and has a population of nearly 175,000 residents, including over 12,000 veterans. Each year, HealthWest services approximately 5,000 individuals with Serious Mental Illness (SMI), Serious Emotional Disturbance (SED), Substance Use Disorders (SUD) and Co-Occurring Disorders (COD). Through this project, HealthWest is committed to serving a total of 2,000 individuals per year using CCBHC award funds. HealthWest's overall CCBHC project strategies are to use current technology and evidence-based practices and programming to eliminate barriers to treatment and improving access to services. All project strategies are woven within a consistent focus on integrating behavioral and physical healthcare, emphasize the use of data within continuous quality improvement to bolster the value and quality of care provided, and demonstrate a commitment to fulfilling CCBHC criteria.
The state project goals are to: reduce barriers to services in Muskegon County via use of community-based service deliver methods, such as telehealth, mobile response teams, and certification of translators and interpreters: expand and enhance existing services to meet growing need for substance use disorder treatment: meet growing demand for youth services by adding resources to HealthWest's Mobile Response and Stabilization Services (MRSS) ream; expand existing services including outpatient, crisis stabilization, and brief intervention treatment; promote integrated behavioral and physical healthcare and improve health outcomes through screening for primary care, active sharing of health information and referrals, and care coordination with PCP's; and ultimately strengthen organizational infrastructure through investments in health information technology, enhanced EHR implements, data analysis and reporting, technical assistance, workforce development and training.
