The CHR-P Project will implement a stepped-care model to reduce the duration of untreated psychosis for individuals under the age of 25 in Travis County (Austin), Texas. This project will implement evidenced-based screening and assessment utilizing the Prodromal Questionnaire-Brief and the Structured Interview for Prodromal Symptoms (SIPS) and expand services to include a second Coordinated Specialty Care (CSC) team. Travis County had a population of 1,024,266 based on the 2010 census. In the past 7 years, the county has seen a drastic increase in its population; it is estimated to have grown 1,226,698 (an 18.7% increase) in 2017. Travis County's population includes 165,903 youth ages 15-24. We can expect approximately 1,660 will experience a first episode of psychosis. 34% of the population identifies as Hispanic/Latino and 32% of individuals speak a language other than English at home. In order to achieve our overarching goal of reducing the duration of untreated psychosis, we will implement the following goals and objectives: Goal 1. Implement elements of a Stepped-Care Model for first episode psychosis in Travis Count. Objective 1a. Hire Clinical Coordinator to provide quarterly training on prodromal and early psychosis warning signs to child-serving system organizations in Travis County. Objective 1b. Hire community-based therapist/screener to decrease referral-to-assessment time to 7 days and provide monitoring of Ultra High Risk (UHR) clients. Objective 1c. Include Prodromal Questionnaire-Brief (PQ-B) in standard Integral Care intake. Plan to screen 300 new clients ages 16-25 annually (1200 total). Objective 1d. Establish UHR monitoring program (of at least 180 days) for new clients who are determined to be at high risk at intake (80 annual, 320 total). Objective 1e. Conduct brief Prodromal Questionnaire-Brief (PQ-B) for all currently enrolled 16 and 17 year olds (100 per year, 400 total). Objective 1f. Establish UHR monitoring program (monitor length of one year) for youth and adults currently involved in programs (30 annual, 120 annual). Goal 2. Increase capacity for Coordinated Specialty Care (CSC) team in Travis County. Objective 2a. Hire additional staff for CSC to reduce waitlist time to 30 days and increase caseload capacity to 45 individuals. Objective 2b. Implement Structured Interview for Prodromal Syndrome (SIPS) and evaluate at least 15 individuals in Year 1 and 45 individuals in each of the following years (150 total). Outcomes will be tracked on individuals who are monitored as UHR or enrolled in CSC (260 individuals). Goal 3 Ensure strategies and structures are informed by System of Care values and principles. Objective 3a. Establish youth and family driven quality improvement committee to assess processes, policies, and outcome data at least quarterly. Objective 3b. Expand meaningful family and youth partnerships and strengthen cultural and linguistic competencies throughout all early psychosis programming.
