South Carolina Department of Mental Health Treatment for Adults Experiencing Homelessness in South Carolina Program Abstract The South Carolina Department of Mental Health (SCDMH) will implement the Treatment for Adults Experiencing Homelessness in South Carolina program using SAMHSA evidence-based practices to provide intensive treatment services and benefits assistance for individuals with serious mental illnesses and co-occurring disorders who are experiencing homelessness. Integrated, comprehensive behavioral health, housing support, employment development, peer, and other recovery-related services will be delivered by two full fidelity Assertive Community Treatment (ACT) teams in Richland and Greenville counties. These counties have the highest concentrations of need for this population of focus in the state. Other evidence-based practices to be implemented by both teams include Individual Placement and Support (IPS) and Integrated Dual Disorder Treatment (IDDT). The Richland County ACT team is a partnership between SCDMH and Palmetto Health, the largest health care system in the Midlands. The Palmetto Health ACT team has a maximum case load of 75 individuals according to ACT fidelity standards. Based on the average client attrition rate, the Richland County ACT team will enroll an estimated 33 additional clients over the grant term. The Greenville County ACT team is operated by the Greenville Mental Health Center, the public mental health agency in Greenville. The Greenville County ACT team has a maximum case load of 61 individuals according to ACT fidelity standards. Based on average attrition rate and fidelity standards, it is estimated that 42 additional clients will be enrolled over the grant term. Both teams will therefore serve a total of 75 additional clients over the five year grant term with full fidelity ACT services in the highest need counties. This program will also add four SSI/SSDI Outreach, Access, and Recovery (SOAR) Benefits Specialist positions, one based in each of the four Continuum of Care regions in the state. One of the positions will be based at the South Carolina Department of Corrections to assist offenders with serious mental illnesses with SSI/SSDI applications prior to their release. The other three positions will be based at local community mental health centers and will partner with local PATH outreach teams to leverage their resources and expedite access to mainstream benefits for eligible clients. Each SOAR Benefits Specialist will submit a minimum of twelve SSI/SSDI applications in year 1 and a minimum of 24 applications each year in years 2-5 for a combined total of 432 submitted SSI/SSDI applications. Access to income supports, such as SSI/SSDI, and health benefits improves housing and treatment outcomes.
