Serving Children and Adults in Need (SCAN), Inc. is proposing to implement its Hogar Para Todos Project (A Home for Everyone) in Webb County, Texas, situated along the Texas-Mexico border. The purpose of the project is to expand the infrastructure in Webb County to integrate behavioral health treatment, primary care and recovery support services, along with housing and other services to meet the comprehensive needs of male and female adults with a serious mental illness or co-occurring disorders who are experiencing homelessness. The Project will serve 300 unduplicated participants in the 5 years of the project. HOGAR will implement four evidence-based practices to address the mental health, substance use and trauma needs of project participants: Motivational Interviewing, Acceptance and Commitment Therapy (ACT), the Matrix Model and Seeking Safety. The goals of the project are: 1) To develop and implement a trauma-informed project that addresses the multiple needs of homeless individuals in an integrated fashion; 2) To improve participants’ lives by demonstrating positive outcomes in several life areas at discharge and 6-months follow-up. 3) To ensure the Project integrates stakeholder and participant feedback into implementation and evaluation of the project; 4) to deliver Evidence-Based Treatment Interventions with High Degree of Fidelity; 5) To develop a cultural and linguistic informed project to meet the specific cultural and linguistic needs of participants from the border region and 6) To collect quality data effectively and consistently to ensure best clinical outcomes, timely reporting and program improvement. The type and numbers of services consist of the following: community outreach and screening and comprehensive assessment; individual, family and group counseling; peer recovery support groups; case management; wrap-around team meetings; linkages to continuing care; peer recovery coaching and peer recovery services; mental health counseling; psychiatric care; transportation; integrated mental health, substance abuse, and trauma treatment; HIV and Hepatitis C testing and counseling; linkages to sustainable housing; assistance in enrolling in health insurance programs; health screenings with linkages to primary care treatment. The Project will lead the development of a Stakeholder Group composed of local agency representatives, participants and other stakeholders to meet and monitor the goals outlined in the program. The Project will also form Participant and Family Advisory groups to obtain direct feedback from participants and significant others about the quality of services.
