The City of Pasadena Public Health Department (PPHD) requests $900,000 to support the Pasadena Outreach Response Team (PORT). PORT was launched in late 2018 and is a partnership between the PPHD and the Pasadena Fire Department, as well as Union Station Homeless Services and Huntington Health. PORT is a multi-disciplinary team that includes a social worker/case manager, firefighter, peer navigator, and nurse. The requested funding will be used to support members of the existing team, as well as expand the team to include outreach workers to further engage potential clients and an LCSW to provide field-based mental health and/or substance use assessments, linkages, and referrals. Funding will also be utilized to increase access to basic needs services for clients, including showers and laundry. The intent of PORT is to provide supportive services and wraparound case management to move more people experiencing homeless off the street and into housing. PORT takes special focus on the most high acuity and hard to reach homeless population: those suffering from substance use disorders (SUDs) or co-occurring mental health conditions and substance use disorders (CODs). PORT provides intensive case management, linkage to SUD or COD treatment, and support to permanent housing. PORT is a critical component of the City’s broader effort to combat homelessness which includes ongoing and targeted data collection on the City’s homeless population, strategic planning and coordination, and efforts to increase the inventory of transitional and permanent housing.
