Kauhale After-Care Recovery Program - The YMCA of Honolulu's Kalihi YMCA Branch Kauhale After-Care Recovery Program is a 12-session substance abuse recovery program geared towards middle and high school aged youth who have been clinically discharged from a substance abuse treatment program. The geographic catchment area includes Makiki/Moiliili through Kalihi/Moanalua/Aiea, Waipahu, Ewa Beach, Kapolei, Wahiawa, Waialua, and the Leeward Coast, all in O'ahu, Hawaii. The Kalihi Y Kauhale After-Care Recovery Program will: 1. Provide opportunities for youth discharged from a substance abuse treatment program to receive further after-care services to re-engage back into their community to remain clean and sober. 2. Create an after-care recovery wellness plan with youth to identify and practice refusal and coping skills which they can utilize in their community and home environment. The Kauhale After-Care Recovery Program consists of evidence-based practices for youth who have been successfully discharged from a Substance Abuse Treatment Program and are being referred for after-care recovery services. The primary source of referral to the program will be from the Kalihi YMCA's school-based substance abuse treatment program, as well as other adolescent substance abuse treatment programs in the area. The Kalihi Y currently provides substance abuse treatment to more than 20 middle and high schools in the area. The program will utilize an evidence-based curriculum based on Living in Balance, Building the Beloved Community, and the Stages of Change. With the support of the YMCA's Kauhale Program, youth will have the opportunity to utilize and practice the skills learned within their previous treatment program to lead a clean and sober lifestyle as they re-engage in their community. Individual/group sessions and positive youth development activities will take place in the youth's home communities. Sessions and curriculum will focus on building support systems and networks, continuing motivation for change after treatment, and creating and following an after-care recovery wellness plan. Objectives: 1) By September 29, 2023, at least 125 eligible youth with register in the Kauhale After-Care Recovery Program. 2) By September 29, 2023, at least 94 youth will complete the 12-week program sessions. 3) By September 29, 2023, 25 percent of youth completing the program will participate in a career/higher education exploration program, be involved in an internship or volunteerism, or have a job. 4) Upon discharge from the Kauhale program, 50 percent of youth completing the program will be involved in a positive and supportive program/activity to support sobriety. Activities/programs include: Sports, Clubs, and other extracurricular activities, YMCA Memberships, etc. Direct services will be provided to 94 unduplicated youth with these grant funds. This is the CDS Project: YMCA of Honolulu, HI, to provide behavioral health care services and counseling to youth.
