Ryan White Part C Outpatient EIS Program - Founded in 2005, Quality Home Care Services (QHCS) d/b/a Quality Comprehensive Health Center (QCHC) is a non-profit 501(c)3 organization serving a multi-county region that touches seven designated Medically Underserved Areas (MUAs) in and surrounding Charlotte, North Carolina. QCHC offers one of the largest internal HIV continuum of care in the Charlotte Transitional Grant Area (TGA) which includes counties throughout North Carolina and one in South Carolina, including Mecklenburg, Union, Cabarrus, Anson, Gaston and York (SC). The mission of QCHC is to provide competent, committed, consistent, and compassionate support to all we serve. The vision is to be the premier agency responsive to the stigma of HIV/AIDS, and intellectual/developmental and physical disabilities by increasing inclusive practices of program and professional integrity to the greater Mecklenburg community. QCHC has been implementing RWHAP services and programs since 2007 and is a former recipient of RWHAP Part C EIS funding. QCHC is proposing to provide all five RWHAP Part C Early Intervention Services (EIS) directly through its internal HIV Continuum of Care. These services include: (1) counseling for individuals with respect to HIV; (2) targeted HIV testing; (3) periodic medical evaluations of individuals with HIV and other clinical and diagnostic services regarding HIV; (4) therapeutic measures for preventing and treating the deterioration of the immune system, and for preventing and treating conditions arising from HIV; and (5) referrals for people with HIV to appropriate providers of health and support services. QCHC currently offers a range of RWHAP services through its existing Part A MAI and Part Capacity Development funds onsite at its one stop shop health care campus. QCHC will leverage its existing case management program, extensive referral network already in place, and staff to implement the proposed RWHAP Part C program. Target Population for RWHAP Part C EIS services: Black/African American females, Hispanic/Latinx MSM, and transgender women. The target populations were chosen because of HIV risk and prevalence in the service area as well as to ensure no duplication of effort. QCHC will use Part C funds to complement QCHC’s current CDC-funding HIV prevention/testing award which has a focus population of African American MSM.
