The Part C Clinic has served western Colorado since 2000. General overview of the HIV epidemiology in the entire designated service area selected: For the proposed project, which is executed at St. Mary’s Regional Hospital, will serve the following counties: Archuleta, Delta, Dolores, Eagle, Garfield, Grand, Gunnison, Hinsdale, Jackson, Lake, La Plata, Mesa, Moffat, Montezuma, Montrose, Ouray, Pitkin, Rio Blanco, Routt, San Juan, San Miguel, and Summit. The number (prevalence) of unduplicated PLWH in the service area has increased from 716 to 780 from 2021 to 2023, an increase of 8.9%. There continues to be a steady number of newly diagnosed people in the service area. There were an average of 22 newly diagnosed people per year in the service area. In 2023, the most common form of transmission for PLWH in the area is men who have sex with men (MSM), at 61.4 %, followed by heterosexual transmission (13%), people used drugs intravenously made up 6.7% of the population and pediatric cases made up 0.7% of new cases. Transfusion/hemophilia related cases were 1% of new diagnoses. People with unknown risk made up 10.2% of new diagnoses. Description of the key services to be supported by this request, the amount requested, and the target populations (including sub-populations) to be served: The Part C clinic offers key HIV services, including early intervention and linkages to care and core medical and support services (mental health services, oral health care, outpatient ambulatory health services, substance use disorder outpatient care, medical case management, care coordination, antiretroviral prescription and monitoring, and referral to partnering organizations for specialty care not provided by the clinic). The Part C clinic requests $341,850 for each year of the three-year period of performance. The target population for the Part C clinic is all current and prospective people living with HIV (PLWH) in Western Colorado. The clinic’s patient population increased by over 300%, going from 71 unduplicated individuals to 300, since its inception 25 years ago. Targeted subpopulations to be served are MSM, women, racial and ethnic minorities, and people with a new diagnosis.
