Ryan White Part C Outpatient EIS Program - Five Horizons Health Services (FHHS) is a private, non-profit, community-based AIDS Service Organization that was established in 1988. FHHS has been a Ryan White grantee for the last 30 years. In 2018, FHHS opened a medical clinic to begin providing medical services to our service area. FHHS strives to provide high-quality, patient-centered medical, prevention, supportive, research, and advocacy services. In our Southern region, we currently operate five permanent clinics located in Montgomery (2), Selma, Dothan, and the Beacon Center, along with seven rural clinics in Clayton, Andalusia, Linden, Selma, Troy, Enterprise, and Eufaula, including ADPH sites. Within these twelve clinics, we provide a comprehensive continuum of outpatient HIV primary care services including social services, medical treatment, medication assistance, pharmacist consultations, mental health counseling, patient education, prevention education, HIV testing, and interpretation services (Spanish/English) for the hearing impaired. In 2023, FHHS’ Southern Region served a total population of 2,477 patients in 18,675 mostly rural square miles, which is nearly 1/3 of Alabama's geography. Currently FHHS’ Southern Region patient population is 75 percent African American; 20.3 percent White; 3.5 percent Hispanic; 0.4 percent Asian, and 0.7 percent other. Sixty-seven percent (67%) of our patients are male and thirty-two percent (32%) female. We have thirty-one transgender patients. Of the 1,677 male patients, 60 percent are men who have sex with men (MSM). Many of the FHHS patient population reside in areas that are extremely rural, have no public transportation, and little or no access to healthcare. This project will serve a 26-county area which includes the following counties: Autauga, Barbour, Bullock, Butler, Chambers, Chilton, Coffee, Conecuh, Covington, Crenshaw, Dale, Dallas, Elmore, Geneva, Henry, Houston, Lee, Lowndes, Macon, Monroe, Montgomery, Perry, Pike, Russell, Tallapoosa, and Wilcox. This project will provide comprehensive HIV primary healthcare and needed social services for people living with HIV (PLWH) residing in rural, medically underserved areas within FHHS’s 26 county Southern Region service area. In addition, this project also addresses the four goals of the National Strategic Plan: A Roadmap to Ending the Epidemic: prevent new HIV infections; improve HIV-related health outcomes for people with HIV; reduce HIV-related disparities and health inequalities; and achieve integrated, coordinated efforts that address the HIV epidemic among all partners and stakeholders.
