CAPWN is requesting Ryan White HIV/AIDS Program (RWHAP) Part C Early Intervention Services (EIS) funding to provide comprehensive HIV primary health care and support services in an outpatient setting to the 11 western most counties of Nebraska. CAPWN has provided services to individuals with HIV since 1996 and has been a recipient of RWHAP Part C EIS funding since 2023. The CAPWN RWHAP Part C EIS program (CAPWN Part C program) is one of the smallest Part C programs in the country, however, its official service area covers 14,095 square miles, an area slightly larger than Massachusetts, Connecticut and Rhode Island combined. In addition to providing services to individuals in the official service area, the CAPWN Part C program provides care to several individuals with HIV who reside in eastern Wyoming which adds several thousand additional square miles to its unofficial service area. The CAPWN Part C program service area is rural and sparsely populated. Seven of its 11 counties have a “Frontier” designation (less than six people per square mile). There are no other RWHAP funded programs in the service area. The nearest RWHAP funded programs are in Cheyenne, Wyoming, 100 miles to the west; Rapid City, South Dakota, 200 miles to the north; and in Ft. Collins, Colorado, 150 miles to the south. The CAPWN Part C program service area is a low incident area for HIV infection. According to the Nebraska Department of Health and Human Services (DHHS) surveillance data report, a total of six (6) individuals have been newly diagnosed with HIV in the service area in the past three years (2021 – 2023). Of those six individuals, five of them were linked to care to the CAPWN Part C program within three months of care. The primary mode of HIV transmission in the CAPWN Part C service area is Men who have Sex with Men (MSM). When compared to the general population demographics of the service area Hispanics/Latinx and American Indians are overly represented in the CAPWN Part C program as well as in the DHHS surveillance data for the service area. Based on the DHHS surveillance data and the historical knowledge of providing medical services to individuals with HIV in the service area, the target population for the CAPWN Part C program are ALL individuals with HIV living in the service area with an emphasis on individuals with HIV in the Hispanic/Latinx and American Indian communities. The CAPWN Part C program collaborates with a number of providers in the region to ensure a comprehensive set of health care and support services are delivered to individuals with HIV in the service area. As a FQHC, CAPWN provides a robust set of health services to its patients including ambulatory/outpatient care, oral health care, mental health care, substance misuse care, medical nutritional therapy, reproductive health and medical case management. In addition, the CAPWN Part C program provides cost sharing assistance for low-income individuals, health education/risk reduction, medical transportation, and outreach services. In addition to the internals services it provides, to ensure a comprehensive set of Early Intervention Services are available to those enrolled in the Part C program, the CAPWN Part C program also contracts with several other providers through the Part C program to ensure a comprehensive set of Early Intervention Services.
