Ryan White Part C Outpatient EIS Program - The Costal Bend Wellness Foundation (CBWF), located in Corpus Christi, Texas, is a nonprofit Federally Qualified Community Health Center and a Certified Community Behavioral Health Clinic, serving the Texas Coastal Bend area. CBWF is the only Ryan White, Part B HIV service provider in the area offering prevention services and treatment of HIV disease and has forged partnerships with local, state, and federal funding sources to better serve at-risk and HIV-infected persons. CBWF has developed a “one-stop-shop” service model that includes street outreach, HIV counseling/testing, case management, HIV early intervention services, clinical services, STD screenings, primary care, psychiatric services, mental health counseling, dental care, Medication-Assisted Treatment (MAT), substance abuse treatment, mental health counseling and transportation services. Collectively, these services establish a continuum of care for the at-risk and HIV-infected persons living in the catchment area. CBWF is proposing to remain a Ryan White, Part C Provider and servicing a geographic area that includes the counties of Nueces, Aransas, San Patricio, Kleberg, and Jim Wells. This area does not overlap with any existing Ryan White HIV/AIDS Program (RWHAP) Part C EIS. CBWF provides comprehensive outpatient primary health care and support services to primarily minority, low income, uninsured, and underinsured people living in the Coastal Bend. CBWF proposes the following: 1. counseling for individuals with respect to HIV; 2. targeted HIV testing; 3. medical evaluation, clinical, and diagnostic services; 4. therapeutic measures for preventing and treating the deterioration of the immune system and for preventing and treating conditions arising from HIV/AIDS; and 5. referrals to appropriate providers of health care and support services.
