Ryan White Part C Outpatient EIS Program - Project Abstract Pennsylvania Western University operating as PA Thrive Partnership is a RWHAP Part B and RWHAP Part C grant funded HIV clinical mission that has served the Northwest Pennsylvania region for over 30 years. We continue to use our RWHAP Part C funding to pay for health care professionals that are specialists in the field of HIV health care. Funded by our RWHAP Part C grant are 1 Ryan White Clinic Supervisor who is a registered nurse, 1 nurse practitioner, 2 registered nurses, and 1 infectious disease physician that specializes in HIV clinic care. We provide Early Intervention Services and Core Medical Services within our clinics located in Clarion, Crawford, Venango, Lawrence, Erie, Warren, and Clearfield counties in Pennsylvania. Our Part C clinic provides HIV testing for partners of current patients—with our High Impact Prevention Team, funded by Pennsylvania State funding, providing linkage to our clinic for patients newly diagnosed as HIV positive within our region. Part C patients are also case managed by our registered nurses and given additional health education and risk reduction education. Part C clinic HIV specialists also counsel patients in any additional needed health care coverage and assist any patients that may need Pennsylvania’s ADAP program called Special Pharmaceutical Benefits Program, or SPBP. Ambulatory Outpatient Health Care is provided for our patients by our mobile clinic model that brings our services to the counties mentioned above, with PA Thrive having 3 brick and mortar locations in Erie, PA (Erie County), Clarion, PA (Clarion County), New Castle, PA (Lawrence County), and Dubois, PA (Clearfield County). Partnerships with county Departments of Health allow us to bring clinic services to Crawford, Venango, Warren, and an additional space in Clarion County. Supplemental services to our core services are provided using RWHAP Part B funding that include medical nutrition therapy, additional medical case management services, emergency funding assistance (including health care insurance premiums), linguistic services, and food bank services. Additionally, we continually work to improve our programs through Clinical Quality Management measures and Continuous Quality Improvement projects based on unmet needs of our patients. Through our Consumer Advisory Board (CAB), we are directly communicating with our patients and learning what services, beyond those currently provided, are needed to improve the health outcomes of our patients. Through the RWHAP Part C grant, we will continue our mission of providing high quality health care for people living with HIV in the Northwest Pennsylvania region.
