AHF will provide CTR services, linkage to primary health care and support services, referral to care and supportive services, retention in care, monitoring viral suppression/ART, outpatient/ambulatory health services, medical case management, treatment adherence services, risk reduction counseling, and a range of support services. The project will serve the Pinellas County, FL. Needs to be addressed: AHF’s project will address gaps in the local HIV care continuum on diagnosis of HIV infection, linkage to care, retention in care, Receipt of ART, and the achievement of viral suppression. The number of PLWH in Pinellas County has increased, from 4,853 PLWH and a rate of 495/100,000 in 2019 to 5,166 PLWH and a 532 rate in 2022. In Pinellas County, the HIV incidence rate (per 100,000) was 12.6 (122 cases) in 2022; the rate was 12.5 in 2021 and 15.5 in 2020. The incidence rate of AIDS diagnoses (per 100,000) was 7.0 (68 cases), compared with the statewide rate of 8.9; the rate was 7.2 in 2021 and 8.1 in 2020 African Americans accounted for 40 of the 122 (33%) of newly reported HIV cases in Pinellas County in 2022, and 23 of the 68 (34%) newly reported AIDS cases, showing the need for increased access to testing and treatment. In the service area, 33% of newly diagnosed persons are black, despite comprising just 10% of the overall population. Fully 1,522 of the 5,166 (29%) PLWH in Pinellas County were Black, over three times the overall population. HIV disparities in women are particularly pronounced as African American women represent 59% of all female HIV cases each year in Florida, despite making up 16% of the female population statewide. In the service area, MSM have been most impacted by HIV, accounting for 61% of new cases and 66% of PLWH, while heterosexual transmission accounted for 20% of new cases and 20% of PLWH. Proposed services: Key services will include: linkage to primary health care and support services, referral to care and supportive services, retention in care, monitoring viral suppression/ART, outpatient/ambulatory health services, medical case management, treatment adherence services, risk reduction counseling, and a range of support services. Population groups to be served: AHF’s target populations—PLWH in the designated service area of Pinellas County—are underserved and experience disproportionately poorer health outcomes; the specific subpopulations that have the greatest needs for receiving RWHAP Part C funded services include minority populations (Black and Hispanic), men who have sex with men (MSM), Transgender individuals, drug users, recently incarcerated persons, and women of color; AHF will also target PLWH currently not in care. AHF will target its services in southern Pinellas County, an underserved area with a large African-American population.
