AHF will provide CTR services, linkage to primary health care and support services, referral to care and supportive services, retention in care, monitoring viral suppression/ART, outpatient/ambulatory health services, medical case management, treatment adherence services, risk reduction counseling, and a range of support services. The project will serve the Palm Beach County, FL. Needs to be addressed: AHF’s project will address gaps in the local HIV care continuum on diagnosis of HIV infection, linkage to care, retention in care, Receipt of ART, and the achievement of viral suppression. The service area experiences rates of HIV/AIDS that have increased since 2020. In Palm Beach County, the HIV incidence rate (per 100,000) was 20.3 (309 cases) in 2022, compared to the rate of18.4 in 2021 and 13.2 in 2020. The incidence rate of AIDS diagnoses (per 100,000) was 9.8 (149 cases), exceeding the statewide rate of 8.9; the service area rate was 7.7 in 2021 and 6.1 in 2020. In 2022, there were 8,676 PLWH in Palm Beach County, for a rate of 571/100,000 (0.57%), exceeding the statewide rate of 558. In the service area, 53% of newly diagnosed persons are Black, despite comprising just 17% of the overall population. Fully 4,972 of the 8,676 (57%) PLWH in Palm Beach County were Black, more than three times their overall population. In the service area, males have been most impacted by HIV, accounting for 73% of new cases and 66% of PLWH; 110 of the 149 (74%) new AIDS cases in 2022 were in men. The primary method of transmission for African American women in Florida is heterosexual sex, accounting for 96% of all HIV cases, with 3% IDU. In the service area, heterosexual transmission accounted for 46% of new cases and 51% of PLWH, with MSM accounting for 42% of new cases and 38% of PLWH. Proposed services: Key services will include: linkage to primary health care and support services, referral to care and supportive services, retention in care, monitoring viral suppression/ART, outpatient/ambulatory health services, medical case management, treatment adherence services, risk reduction counseling, and a range of support services. Population groups to be served: AHF’s target populations—PLWH in the designated service area of Palm Beach County—are underserved and experience disproportionately poorer health outcomes; the specific subpopulations that have the greatest needs for receiving RWHAP Part C funded services include minority populations (Black and Hispanic), men who have sex with men (MSM), Transgender individuals, drug users, recently incarcerated persons, and women of color; AHF will also target PLWH currently not in care.
