To fulfill this mission, LPH established the Department of HIV/AIDS in 1985 to address the growing concerns of HIV along the U.S. and Mexico Border. LPH has forged partnerships with local, state, and federal funding sources to better serve at-risk populations and People Living with HIV (PLWH). Over the past three decades, LPH has developed a “one-stop-shop” service model that includes outreach and prevention, HIV testing/counseling, HIV early intervention services, STD screenings and treatment, case management, medical care, basic dental care, mental health, housing assistance, and transportation services. These services establish a continuum of care for at-risk HIV-infected persons living along the U.S./Mexico Border. The target area is located within Texas Health Region 11, covering 4 counties along the Texas-Mexico Border: Jim Hogg, Webb, Starr, and Zapata with a total population of 351,495 (U.S. Census Bureau, 2022 Population Estimate of Texas by County for Population). Out of 254 counties in the state, Webb County, one of the largest targeted counties within the area, holds the 24th place within the state with the highest HIV infection and 22nd in AIDS case rates (Texas HIV Surveillance Report; TDSHS, 2021). This is significant considering the area is mostly made of rural communities. The LPH HIV/AIDS Program is proposing to continue being a Ryan White Part C Provider and servicing a geographical area that includes Webb, Zapata, Jim Hogg, and Starr Counties – this area does not overlap with any existing RWHAP Part C Early Intervention Services (EIS). LPH will provide comprehensive outpatient primary health care and support services primarily to minority, economically disadvantaged, uninsured, and underinsured PLWH along the U.S. and Mexico Border. Additionally, the program will focus on increasing linkage to care for vulnerable populations such as people who inject drugs, transgender individuals, MSMs, female sex workers, and pregnant women. The program will continue to focus on increasing the number of persons from racial and ethnic minority populations, primarily Latinos identified with HIV infection, and increasing their linkage to HIV care. LPH will provide HIV positive persons with the following Core services: Early Intervention Services, Outpatient/Ambulatory Health Services, Dental, Mental Health, and Medical Case Management. LPH will also provide the following Support Services: Health Education/Risk Reduction, Non-Medical Case Management, Client Transportation, and Outreach services. LPH is requesting $932,397 for FY 2025-2028.
