The service area is composed of Boyd, Carter, Fleming, Greenup, Lawrence, Lewis and Mason Counties in); Adams, Athens, Brown, Clinton, Gallia, Jackson, Lawrence, Meigs, Pike, Ross, Scioto, Vinton, and Washington Counties in Ohio and Cabell, Jackson, Mason, Wayne, and Wood Counties in West Virginia. Key services to be provided include: • HIV testing using the Counseling, Testing, and Referral model • linkage to care using ARTAS • core medical services include outpatient ambulatory care, mental health care, substance use disorder treatment, and medical nutrition therapy • support services include medical transportation, food bank/home delivered meals, outreach services, and emergency financial aid. The goal of all services is to increase retention in care and viral load suppression. By providing these services, the project will achieve anti-retroviral prescription rates of 80% and viral load suppression of 75% This grant proposal requests $291,135 for the period January 1 to December 31, 2025 with two renewal periods for an entire project period of January 1, 2025 to December 31, 2027. Target populations for this Ryan White Part C EIS project are people living with HIV in the targeted region who are un-insured or under-insured, and have annual incomes below 500% of the Federal Poverty Level. Priority populations for the project are men who have sex with men (MSM) and people who inject drugs (PWID)
. Sub-populations within these priority groups are individuals who do not know there are living with HIV and people who know they have HIV but are not linked to care.
