(formerly Hudson River HealthCare), a New York State licensed, federally qualified health center (FQHC) recognized by the National Committee for Quality Assurance (NCQA) as a Patient Centered Medical Home, is seeking support to maintain and strengthen its successful Genesis program of HIV care, treatment, and supportive services in in Suffolk County, NY. • Needs to be addressed – Sun River Health is proposing to serve its existing Ryan White Part C service area, the County of Suffolk, one of the two New York counties which make up Long Island. The New York State HIV/AIDS Surveillance Annual Report for Cases Diagnosed through December 2022 (most recent report available) reveled that, in 2022, there were 2,895 individuals with HIV in Suffolk, including 100 who were newly diagnosed in that year. In addition, the New York State Department of Health HIV Cascade of Care data for 2022 (most recent year available) estimates that, in the Nassau-Suffolk Ryan White Region, 8% of individuals with HIV are unaware of their status and, of those with diagnosed HIV, only 89% are receiving HIV care; this means that there are approximately 480 people in Long Island with HIV who are unaware of their infection and 610 persons with diagnosed HIV who are not engaged in treatment. • Proposed services – Sun River Health is requesting $342,259 to continue meeting the needs and addressing the gaps in HIV care delivery in the Suffolk County. Genesis is a comprehensive, integrated, multi-disciplinary model of HIV care and treatment designed to meet the complex needs of the Health Center’s patients with HIV. It provides high-quality, culturally and linguistically competent primary, preventive, behavioral and oral health care, screening and treatment for substance use disorder, specialty care, as well as a full range of support services and seamless linkage to a highly responsive referral care network, as needed, for those with HIV. The Health Center will use Part C funding to support core medical and support services including outpatient/ambulatory health services; medical case management, including treatment adherence; health education/risk reduction services; and outreach services. • Population groups served – The Health Center’s Genesis HIV treatment program targets individuals with HIV from low-income and medically underserved communities with a particular focus on subpopulations who are disproportionately infected with, and affected by, HIV including African Americans, Hispanics/Latinos, men who have sex with men (MSM), women, people who inject drugs, and individuals experiencing homelessness.
