(doing business as Family Health Centers at NYU Langone) is requesting competing continuation funding in the amount of $618,619 per year for three years (January 1, 2025 - December 31, 2027). Funding Preference: Qualification 1: Increased Burden; and Qualification 3: Underserved Populations The Sunset Park Health Council, Inc. (Family Health Centers at NYU Langone or FHC) is requesting $618,619 in Ryan White HIV/AIDS Program (RWHAP) Part C Early Intervention Services (EIS) funds to support the delivery of HIV core medical and support services in FHC’s six-neighborhood designated service area in Kings County (Brooklyn), New York: 1) Sunset Park, 2) East Flatbush/Flatbush, 3) Park Slope, 4) Red Hook, 5) Brownsville, and 6) Crown Heights. As demonstrated by NYC Department of Health and Mental Hygiene HIV/AIDS Surveillance Data, out of the 31,149 people living with HIV (PLWH) in Brooklyn in 2022, more than half (56%) live in FHC’s service area, revealing a higher HIV/AIDS prevalence (1.9%) than both Brooklyn (1.2%) and NYC overall (1.6%). While Brooklyn’s HIV incidence rate (17.11/100,000) is lower than NYC (19.2/100,000), FHC’s service area has higher diagnosed HIV incidence (23.7/100,000) than both Brooklyn (17.11/100,000) and NYC as a whole (19.2/100,000). The majority of PLWH in the service area are Black (68%) or Hispanic (17.8%), and many are of immigrant status. The service area consists of low-income, REM households with large subpopulations of Hispanic/Latino, Afro-Caribbean, R ussian, Arabic, and Chinese (Mandarin-speaking) immigrants with significant barriers to needed health services. Social determinants of health such as low educational attainment, unstable housing, poor physical conditions of neighborhoods, disproportionate incarceration rates, and patterns of residential segregation all contribute to the high incidence and prevalence of HIV/AIDS, and other poor health sequelae (U.S. Census Bureau, 2022). Through detailed analysis of patients served, FHC has identified the following sub-populations to be the most underserved out of more than 700 clients living with HIV and will target them specifically with RWHAP Part C EIS funding: 1) immigrants; 2) cis gender and transgender Black and Hispanic women over the age of 50; and 3) other newly diagnosed patients who become aware of their HIV status through routine testing. With three decades of experience serving people living with HIV, and a RWHAP grantee since the program’s inception, FHC will provide comprehensive outpatient HIV early intervention and primary care services across the service area (with clinical hubs at Sunset Terrace and Flatbush FHC locations). The following services will be provided, reaching patients at every stage of the HIV Care Continuum: HIV testing and counseling; linkage to care; outpatient ambulatory health services; mental health screening and treatment services; medical nutrition therapy; non-medical case management; and referral to community-based organizations for other health care and support services.
