Increased Burden, Rural Areas and Underserved Populations. Summary of Application. Little River Medical Center, Inc. (LRMC), a non-profit community health center, has been providing accessible, affordable, high-quality, patient-focused primary health care to residents of Horry County and Georgetown County in SC, as well as Brunswick County, NC, since 1978. In 2012, LRMC was awarded RW Part C funding to implement an Early Intervention Services (EIS) program for the growing number of people with HIV (PWH) in the region. In accordance with the Ending the HIV Epidemic initiative, LRMC’s Part C EIS program activities strive to decrease transmission and improve health outcomes for PWH and reduce HIV-related health disparities and health inequities by focusing on the priority strategies of Diagnose, Treat, Prevent, and Respond. Continuation of Part C EIS program funding will ensure that LRMC can continue providing convenient and accessible comprehensive primary care to low-income individuals living with HIV in this region, ensuring access to the essential HIV care and treatment and support services needed to help them achieve viral suppression. LRMC anticipates serving 385 ongoing EIS primary care users in the upcoming year, with approximately five new HIV patients enrolling in care. Service Area and Needs. As of December 2022, there were 1,651 PWH in the LRMC service area. This marks a 5.8% increase (from 1,560) since 2020. There were 842 people, or 51.0%, living with HIV-non-AIDS, while 49.0% (809) were people living with AIDS. The number of people newly diagnosed with HIV is increasing, with 62 new diagnoses in 2022 compared to 44 in 2020 and 51 in 2021. African Americans are the racial group most impacted by HIV infection. In 2022, African Americans made up 40.3% of HIV/AIDS cases in the service area, while accounting for just 13.4% of the total service area population. Whites, who make up 78.6% of overall residents, comprise 44.5% of PWH (SC Department of Health & Environmental Control (SC DHEC) and the NC Department of Health and Human Services, March 2024). Proposed Project, including Key Services, Amount Requested, and Target Populations. Part C EIS program funding (Funding Amount: $356,323) will ensure that LRMC can continue providing convenient and accessible comprehensive primary care to low-income and uninsured PWH in this region. In response to the increasing number of residents living with HIV/AIDS within the service area and the lack of local resources to meet the tremendous needs, LRMC proposes continued implementation of its successful EIS program. The program includes the following strategies specifically designed to address local gaps and barriers, including gaps impacting the HIV Care Continuum: comprehensive, culturally competent primary medical care at convenient times and locations; and staff/resources for infectious disease, nutrition, medical case management, treatment adherence, behavioral health, oral health, patient transportation, and strong linkages with other organizations providing services to HIV/AIDS patients and their families. Target populations include homeless, low-income, uninsured, MSM, and African American residents.
